NYS Health Requirements for the 2019-20 school year

Students entering grades 1, 3, 5, 7, 9 and 11 in the 2019-20 school are now required to have a physical examination completed at that grade level. NYS law also requires that we request a dental health certificate for these grades as well.  

Students entering pre-kindergarten, kindergarten, and grades 1, 2, 5,7, and 11 will need a vision screening for distance and near vision acuity in addition to a hearing screening within 6 months of admission to school.

Female students in grades 5 and 7 and males in grade 9 require a scoliosis screening.

We are encouraging families to schedule these appointments prior to the return to school if you have a child entering one the grades indicated above. 

Please review the immunizations schedule and your child's records with their pediatrician to ensure your child has the required vaccinations for their age and grade.

NYS requirements include BMI and Weight status category as part of this Health Appraisal.  All health care providers have been notified of this. Upon request we are required to report weight status categories to NYS.  Although this report is general numbers, and not specific to the student, you do have the right to request, in writing, that your child’s information not be tabulated in the report.
